The Mobile Maternal Health Wallet

Launched in 2019, the Mobile Maternal Health Wallet (MMHW) aims to improve access to maternal and neonatal care in Analamanga, Madagascar. Designed and implemented in partnership with the Malagasy Ministry of Health and the NGO Doctors for Madagascar, it utilises mobile communications and payment technology to lower financial barriers to care.

With the MMHW, pregnant women can digitally save or receive money, and pay exclusively for maternal health services at participating healthcare providers, using a simple mobile phone. Those without access to a phone only need a SIM card to use the MMHW, which can be inserted into the phone of a friend, family member, or one made available with participating health workers. To incentivize savings, funds added to the wallet are matched at 50%.

Community health workers raise awareness about the importance of antenatal health services and giving birth attended by skilled health personnel. They help users access and save to the MMHW, for which they receive reliable performance-based payments at predetermined milestones - for example each new enrollment - which are sent directly to a dedicated mobile wallet.

Healthcare providers use an offline-first tablet application to register patients and submit claims for the health services they provide. Claims are automatically scanned for fraud before being reimbursed.

Participating healthcare providers benefit from lower non-payment rates, quicker reimbursement, and an increased demand by patients who can cover the cost of care.
