Health financing cocoa farming communities in northern Madagascar

The Salama Mateza project was located in the Sambirano Valley, which is part of the Diana region in northwestern Madagascar. The region is known for its high quality cocoa production and is a home to many farming communities. In partnership with the chocolate manufacturer Max Felchlin AG and the local cooperative Sambirano SA , mTOMADY facilitated access to health insurance for farming families who produce cocoa in the region.

At participating healthcare providers patients are signed up for health coverage provided by the mutual health insurance Tsiharofy. The sign-up works with a simple mobile phone or tablet with the assistance from health care workers. After the patient registered and received care, the health care workers digitally filed the treatment information and sent the claim request to Tsiharofy. The claim was scanned for fraud and reimbursed upon validation.

880 cocoa farmers and their families in the region enrolled in the Salama Mateza project to benefit from reduced healthcare costs. During the two-year implementation phase, more than 8,500 treatments were carried out, protecting at least 57 families from impoverishing health expenditures. The cooperative Sambirano SA launched another project that enables access to healthcare for a wider group of people. Since Salama Mateza showed very positive outcomes and became financially independent after only two years, both projects were merged in mid-2023 and now operate under one roof.
